Does this come with all the mounting hardware I need?

The grille assembly is designed to bolt directly to factory mounting points. Confirm what fasteners are included with your shipment before starting installation — you may need to reuse OEM hardware from your current grille.

Do the LED lights work right out of the box or do I need to wire them?

The LED DRL assembly is built into the grille. Check with the seller on whether the LEDs are pre-wired to a plug connector that matches your truck's harness or if custom wiring is required.

Will this fit if my bumper has been damaged or repaired?

This grille mounts at factory points on your truck frame, not the bumper. As long as your frame and core support are straight and undamaged, the grille will align properly. If your truck has had collision work to the front end, test-fit before final installation.
